Limited Liability Waiver Acknowledgement of the Center’s Faith-based perspective: I acknowledge that the Third Street Education Center is a Christ-Centered (Christian Faith-Based) non-profit organization (you may ask for a copy of our statement of faith). Third Street does not limit, nor discriminate, against anyone based on religious affiliation (see TSEC policy #Y003). I give permission to the TSEC to speak to/about, discuss and/or teach the tenants of the Christian faith, including referencing the Bible as a part of some programming at the Center. I also acknowledge that I have received a copy of the TSEC Statement of Faith. I recognize that I do not have to participate in the faith-based components of TSEC programming. I agree to not teach or speak against the TSEC Statement of Faith in my capacity as a volunteer. Functions and Activities: I understand that participating in programs, service, recreation and other activities of Third Street Education Center is voluntary. Prior to my participation in such activities, I acknowledge that there are certain risks associated with these activities, including, by way of example, physical injury due to activity-related accidents, and physical injury due to transportation-related accidents. Release of Liability: By signing this Permission and Waiver Form, I expressly warrant that this child named above or I, if I am a participant, am capable of withstanding both the physical and mental demands of these activities. I also expressly assume all risks to the child or me participating in the activities, whether such risks are known or unknown to me at this time. I further release the organization, its Board Members, leaders, employees, volunteers, property owners and agents from any claim that my child may have or that I may against them as a result of injury or illness incurred during the course of participation in these activities. This release of liability is also intended to cover all claims that members of the child’s or my family or estate, heirs, representatives or assigns may have against the organization, its Board members, leaders, employees, volunteers, property owners or agents. I further agree to indemnify and hold harmless the organization, its Board members, ministers, leaders, employees, volunteers, property owners or agents from any and all claims arising from my participation in its activities and programs, or as a result of injury or illness of my child during such activities. I hereby authorize the staff at the Third Street Community to act for me according to their best judgment in any emergency requiring medical attention. I hereby waive and release Third Street Education Center and its staff from any and all liability for any injury or illnesses incurred at any facility. This release of liability also gives permission to take photos and permission to release these photos in all types of publications. Our Statement of Faith: The Third Street Education Center is a faith-based/Christ-centered, non-profit organization. We believe the Bible to be the inspired, infallible, and authoritative word of God. We believe that there is one God, eternally existent in three persons: Father, Son and Holy Spirit. We believe in the deity of our Lord Jesus Christ, in His virgin birth, in His sinless life, in His miracles, in His vicarious and atoning death through His shed blood, in His bodily resurrection, in His ascension to the right hand of the Father, and in His personal return in power and glory. We believe that for the salvation of lost and sinful people, regeneration by the Holy Spirit is absolutely essential. We believe in the present ministry of the Holy Spirit by whose indwelling the Christian is enabled to live a godly life. We believe in the spiritual unity of believers in our Lord Jesus Christ. We believe in the sanctity and dignity of all human life. We believe the God wonderfully and immutably creates each person as male or female, and that these two distinct, complementary genders together reflect the image and nature of God. We believe that God created marriage to be exclusively the union of one man and one woman, and that intimate sexual activity is to occur exclusively within that union. The Third Street Education Center’s Governing Board of Directors unanimously affirmed this statement of faith on the 12th Day of September, 2016.
Emergency Medical Treatment Release: I do hereby give my consent and permission for the above named young person to participate in Third Street Education Center’s Youth Development Programs, including any related activities or events. I further release the Center and their paid and volunteer staff from all liability for any injuries or accidents resulting from any sickness, injury, or accident. Treatment for any illness or injury will be the financial responsibility of the undersigned parent or legal guardian. I hereby authorize the official representative of the Third Street Education Center to approve emergency medical or surgical care during any related activities or events in the event the parent or guardian cannot be contacted.
I represent that I am the parent/guardian of the child listed who is under 18 years of age. I have read the above Permission and Waiver Form and am fully familiar with the contents thereof. I give permission for the child named above to participate in the activities of the Third Street Education Center. I hereby consent to the Permission and Waiver From, including the Release of Liability above, on behalf of the child, and agree that this Permission and Waiver Form shall be binding upon me and my estate.
The well-being of all Third Street Education Center program participants is important. Everyone has responsibilities. When I participate in TSEC programs, I agree to: 1. Engage fully with a positive attitude and creative energy. 2. Be courteous to others, even if they’re different from me. 3. Be cooperative. Encourage individuals. Help others. Support teamwork. 4. Be willing to ask an adult for guidance to make good decisions and take appropriate actions. 5. Use language that is respectful to others. (No swearing, harassment, and bullying) 6. Take care of the property I use and assume responsibility for purposeful damage I may cause. 7. Wear appropriate clothing for program activity(s). Follow the specific dress code, if provided. 8. Be present and on time for all parts of the program. 9. Use technology (Computers, iPods, mp3 players, game devices, cell phones, etc.) during free time, or in ways that contribute to the goals of the program. 10. Not engage in Personal Displays of Affection (PDA). (It is distracting and inappropriate.) 11. Know and follow federal, state and local laws that apply to my age. (No use of tobacco, alcohol, illicit drugs, fireworks, or firearms.) 12. Know and follow safety and security policies of the Center including: not leave the program area at any time without permission from the program supervisor; abide by curfew hours; remain in gender specific areas [no boy in a girl’s room / no girl in a boy’s room], and others. 13. Have fun and learn. I have read and agree to the above Code of Conduct. I understand that if I violate any part of this agreement, I may be sent home from the Center I also understand that if I cause personal or public safety concerns, I may lose my eligibility to participate in some future TSEC Youth Development events, and/or, lose my participation privileges
